Built specifically for merchants who need to collect more information at checkout, such as invoicing, EAN numbers, PO references, CVR, Order details or any custom field your business requires. Each field is its own independent block — add as many as you need and place them anywhere in your checkout flow using Shopify's checkout editor.

I'm sure this app is wonderful. It certainly looks great! but it's difficult to see from the app details to tell you that extra fields in the checkout aren't available to stores that aren't on the Shopify Plus Plan.
